This is a sample application to help guide school principals in filling out the grant request. The committee is looking for concise, statistical type information, so keep it simple. It shouldn’t take longer than 10 minutes to fill out and submit. K-12 Support Sample Application Date Submitted: Example: 9/29/2009 School Name: Your school name Principal’s Name: Principal’s name Contact Person’s Name: Contact person’s name (could be the principal, teacher, community school assistant principal, etc.) Contact Person’s Phone: The contact person’s Phone or PX number Contact Person’s Email: The complete email address of the contact person. Payroll Contact’s Name: The name of the person at your school who will be handling payroll for this grant. Remediation, Tutorial, Test Prep, Parent Education: (Check one). All programs must be designed to enhance student achievement or parent education. Please note that grant monies can fund instruction only, not supplies. Description: (Example) We are planning an after school algebra 1 tutoring program to be led by a certified math teacher from our faculty. The teacher will be assisted by members of our honor society and math club who will give individual attention to each student after a concept has been presented by the teacher. Target Population and How they are Identified: The students we have selected consist of about 25 students who have earned a grade of C or less in general math or have failed algebra 1. Expected Statistical Outcomes: At least 60% of the students will pass the post-test measuring 1st semester algebra 1 mastery. Dates & Times: (Example) Tues & Thurs, 3-5pm, starts 10/6/2009, ends 2/6/2010. Hours: Enter the total hours for your program, max 60. Pre-Program Evaluation Method: (Example) We will use an algebra 1 semester exam as a pre-test. Post-Program Evaluation Method: (Example) We will use the same algebra 1 semester exam as a post-test.
